We want your first visit with us to be as efficient and as enjoyable as possible. To save time, our agents will send your New Patient paperwork, through your patient portal. Please complete these forms prior to your appointment. If you prefer to complete them in person, please print the forms below and arrive 30 minutes prior, with your completed forms.

Full Skin Exams:

A complete visual examination of the skin is an important tool to diagnose skin cancer at an early and treatable stage. Please avoid wearing makeup, nail polish, hairspray or hair gel to the appointment to allow for a thorough exam.

Children in the Office:

We love children, and we feel a strong sense of responsibility to keep them safe while in our office. To this end, we do not allow children in the exam room during surgical procedures or full skin exams. Children 13 and older may remain unsupervised in the waiting room. We respectfully request that children under the age of 13 remain at home unless an adult is available to supervise them. We truly appreciate you helping us keep your child safe.
